THE federal government and its partners, the states, will spend about $329 billion this year for Medicaid coverage for 53 million people. The cost of the program has been rising by 8 to 10 percent a year, nearly wrecking state budgets.

Unchanged, Medicaid is expected to cost federal and state taxpayers $2 trillion over the next five years.
